The Military Officers Association of America (MOAA) works with the MetLife Vision Program, so that you and your family members can choose from one of the largest networks of ophthalmologists, optometrists and opticians in the nation. 

It’s crystal clear: you will have more convenience for routine care and services. The network is so important when choosing a vision benefits plan. It is about more than just size; the plan should include the right mix of vision providers for you and your family.

Cost of Coverage: Rates are determined by your state of residence. Get a personalized rate quote below.

Eligibility: As a member of the MOAA, you and your family are eligible for coverage. Your lawful spouse and dependent children under age 26 are also eligible for coverage. To become insured, an enrollment form must be submitted and the required premium contribution must be paid.

Effective Date: Coverage for you and your eligible dependents will become effective on the first day of the month after your enrollment form has been approved and your first premium is received.

Termination: Your Vision Plan protection will not be canceled due to claims and you cannot be singled out for a rate increase. Your coverage continues as long as you pay your premiums when due, keep your MOAA membership, and the group policy remains in force. Your dependent’s coverage will remain in effect as long as your coverage is active, premiums are paid, and they meet eligibility requirements.

Which types of savings are available through the MOAA Vision Plan?

You can receive savings on the following:

  • Eye exam
  • Frames
  • Lined bifocal lenses
  • Progressive lenses
  • Standard corrective lenses
  • Standard lens enhancements
  • Contact lenses (instead of glasses)

When you go to a vision provider within your network, there are value-added features as well:

How do I use my MOAA MetLife Vision benefit?

Whether you choose to see a participating vision care specialist or not, using your vision coverage is simple and convenient.

  • Visit www.metlife.com/mybenefits to find an eye doctor who is right for you.
  • Review your plan coverage before your appointment.
  • At your appointment, tell them you have the MetLife Vision plan. No ID card is necessary.

That is it! MetLife will handle the rest – there are no claim forms to complete when you see a participating vision care specialist.

If you visit an out-of-network office, you will pay in full for the services and eyewear received at the time of your appointment, including taxes. Then you will submit a completed MetLife Vision claim form and itemized receipt to: MetLife Vision; PO Box 997565; Sacramento, CA 95899-7565. Do I have to visit a participating eye care professional to get coverage?

No. You can visit any licensed eye care professional. However, your out-of-pocket costs are usually significantly lower when you stay in network.

Do my dependents have to visit the same doctor that I select?

No, you and your dependents each have the freedom to choose any eye care professional.

Do I need to file a claim?

Not if you visit a participating location. You do not need to file claims if you stay in-network for care. The office will confirm your eligibility, submit the claim and calculate your out-of-pocket costs, if any, at the time of service.

If you visit an out-of-network office, you will pay in full for the services and eyewear received at the time of your appointment, including taxes. Once you are enrolled in the plan, you can also access the new “Find a Vision Provider” feature on MetLife’s Mobile App3. Just search for “MetLife” at the App Store or Google Play to download, then use your MyBenefits log in information to access this feature.
